What is Fungal Meningitis?

Fungal meningitis is a very rare form of meningitis that is not contagious. It is usually the result of a fungus spreading through the blood to the spinal cord. The current outbreak of fungal meningitis is linked to a batch of steroid shots produced by the New England Compounding Center (NECC). Contaminated, filthy conditions in the laboratory of the center are the cause of the fungal contamination.

 

Diagnosis

Meningitis is diagnosed by a “culture,” or growing bacteria from an infected patient’s spinal fluid. The results will show which strain of meningitis the patient has contracted. The strain of meningitis connected to the current outbreak is Cryptococcus neoformans; it is a yeast found in soil throughout the world that typically affects patients with compromised immune systems. The difference in this outbreak is that patients were injected with the fungus in a steroid shot directly into the spine, joint or other internal site.

 

Treatment

The most common treatment for patients with fungal meningitis is an intravenous therapy called Amphotericin B. Treatment time is dependent on the patient’s tolerance and results of tests but can last as long as ten weeks and have serious side effects.

Fungal Meningitis Symptoms

Fungal meningitis has similar symptoms to bacterial meningitis but they are much more subtle and have a longer incubation period. Symptoms may appear one to four weeks after getting a pain injection. Symptoms include:

  • New or worsening headache
  • Fever
  • Sensitivity to light
  • Stiff neck
  • New weakness or numbness in any part of your body
  • Slurred speech
  • Increased pain, redness, or swelling at your injection site

 

The CDC warns patients who have had a spinal steroid shot since May 21, 2012, to call a doctor immediately if they have any of these symptoms.
In addition, the CDC is warning patients who received joint injections of the NECC steroids to be on the lookout for fungal joint infection. Symptoms include:

  • Swelling
  • Increasing pain
  • Redness
  • Warmth at the injection site

 

Patients who have such symptoms should see a doctor for further tests. Doctors likely will collect joint fluid to test it for fungal infection.

Types of Injections

The injections at the center of the outbreak were administered for pain management and administered in the neck, spine and joints. An ophthalmic drug that is used in conjunction with eye surgery and a cardioplegic solution used in open-heart surgery are often part of the recalled pharmacy’s drugs.

Who is at risk?

Patients who have received injections on or after May 21, 2012 with product from NECC have received contaminated shots from certain lots. Patients who have had injections for pain management are part of the contaminated group. Injection sites include: spine, neck, joints and eyes.

Compounding Pharmacy History

Compounding pharmacies have been around for thousands of years in some form. The Middle East is credited with having the first drugstore in Baghdad in 754 A.D. that used alchemy and pharmacy compounding. Compounding pharmacies of the 1800s gave way to the modern pharmaceutical industry. The scale of drugs needed by the public produced the market and set compounding pharmacies to smaller scale production amounts. The modern compounding pharmacy is typically defined as being for a single individual patient only. The purpose is to have a doctor write a prescription for a patient with allergies, dietary needs or a different dosage than the drug manufacturers produce. The most common modern day preparations are:

  • Patients requiring limited dosage strengths, such as a very small dose for infants
  • Patients requiring a different formulation, such as turning a pill into a liquid or transdermal gel
  • Patients requiring an allergen-free medication
  • Patients who absorb or excrete medications abnormally

The recent problems with NECC stem from a lack of regulation of compounding pharmacies. NECC also stepped outside their license and took the role of a drug manufacturer. They are not subject to the intense FDA regulations and circumvent the process. Drugs from compounding pharmacies can be a cheaper outlet of the same drugs large manufacturers produce.
